Student Income Tax Request

Your 2023-2024 FAFSA Application has been selected for verification. To be considered
for federal student aid for the 2023-2024 award year, you must submit all required
documentation below for yourself and your spouse (if applicable). No determination
can be made regarding your financial aid eligibility until all required documentation
has been submitted.

2021 Student Tax Return (two options):

  • Tax Return Transcript (issued by IRS):
    • You may obtain a copy of your 2021 Tax Return Transcript by visiting irs.gov, clicking “Get Your Tax Record”, then clicking either “Get Transcript Online” or “Get Transcript by Mail.”
    • If choosing “Get Transcript Online”, the transcript can be downloaded and printed immediately
    • If choosing “Get Transcript by Mail”, the transcript should be received in the mail within 10-15 days
  • Federal Income Tax Return (Form 1040):
    • You may submit a signed and dated copy of your 2021 Federal Income Tax Return (1040) in lieu of the transcript noted above. 

 2021 W-2/1099 Forms

  • Please submit copies of all W-2 and/or 1099 forms reported on the 2021 Federal Income Tax Return (Line 1).
    • The W-2 forms submitted should total the amount reported on the “Wages, Salaries, Tips, etc.” line.

2021 Tax Schedules (as applicable)

  • You should submit any of the following tax schedules that were included as part of
    your 2021 Federal Income Tax Return:
  • Schedule 1: Additional Income and Adjustments to Income
  • Schedule 2: Additional Taxes
  • Schedule 3: Additional Credits and Payments
  • Schedule C: Profit or Loss from Business
  • Schedule F: Profit or Loss from Farming
  • Schedule K-1 1065: Partner’s Share of Income, Deductions, Credits, etc.
