IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Your Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available for you to access electronically, you can now view and print it in Workday as follows:

·         Login to Workday

·         Select the “Benefits and Pay” Application

·         Click on “View” and select “My ACA Forms” 

If you are unable to access your form in Workday and would like to request a paper copy, you may do so by:

·         Writing to: SLU 10799, Hammond, LA 70402

Paper copies will be provided within 30 days of the request.
